Workers not making Cave Entrance

First, I’m sorry but I don’t have any pics at hand

yesterday I tried to make a town that is mostly underground, and in the first 5 minutes of gameplay, found the first bug:

• first, i made a 4 x 4 x 4 hole in the mountain wall
• then I start building, block by block, a frame around the door, 3 blocks width, like this:

±±±+OOOO±±±+
±±±+OOOO±±±+

where O is the hole in the wall, but they stop at the second level, leaving something like this:

any advice with this?

Scaffolds and low ceilings

Not sure if this has been noted in another topic or if I am the only person that likes to do this, but I like to put a wall with a double door in front of my mine entrance just for the look of things. The trouble is that since I am building the wall a bit higher than the cave entrance I run into a little problem half the time. If the workers build the scaffolding outside the mine then I am good.
