That might be a good continuation of the gong-concept (where you place gongs to make the game harder in the orc campaign), but further along in the storyline.

Also maybe it should have dark ruins of brave hearthlings that tried to settle a town in the realm.
A few houses, a church ith a broken tower, was thinking of the same colour sceme as the necromancers cript.

I don’t know, that kind of happens quickly, if you let it start at the start of a game, (assuming things like night, and tree shadows also qualify as “in the dark”). However i think i have an idea for that, in a way that is in line with the storyline of this portal that you open with the cultists.
If the opening of a portal is a big quest in and off itself, whereby you can get progress (think the christmas tree in frostfeast mod), maybe portal shards, or dark gemstones that need to be placed on pedestals arranged in a significant configuration. Anyway, in such a case you can introduce the darkrealm effects into the overworld as soon as the first bit of progress on the portal is done, increasing the severity and intensity of the dark forces every step of the way, as you connect the two worlds together. This then all culminates into a full passable connection between the two worlds (portal), through which not only forces and effects can travel, but also objects and beings.

i guess with normal they come out when the portal is opened and Hard they come out with any darkness after 5 days, ill update original with something else
I could honestly see the nightmare realm to be a biome you could start in, with it’s own set of creatures and monsters, trees, resources and it’s very own campaign. You could even have it be where a cultist is the only one who would have the courage to fight the monsters in this world making it essential.
The cultist being the only one that can fight them is a good idea, but if you start in it would your hearthlings have went there for something? were they born there? how did they get there dude
I guess that depends on if it’s an alternate plane of existence or just a region of the world called the nightmare realm. If I were a settler looking to start my life off as far away from the kingdom thats made me want to leave it might sound appealing.
Keep in mind stonehearth’s world doesn’t have to follow the entirely same logic that our world does.
